The Long-Acting Injectable (LAI)/PrEP Coordinator supports the delivery of high-quality, accessible, and affirming sexual health services involving long-acting injectable medications. This position coordinates patient appointments, medication access, clinical workflows, follow-up care, documentation, and communication among patients, providers, pharmacies, laboratories, and insurance partners.
The Long-Acting Injectable (LAI)/PrEP Coordinator plays an essential role in helping patients initiate and remain engaged in long-acting injectable treatment or prevention services, including HIV prevention and treatment, STI-related care, and other applicable sexual health therapies. The ideal candidate is organized, compassionate, culturally responsive, and committed to reducing barriers to care.
Essential Responsibilities
- Support clinic flow on injection days, including patient check-in, documentation, care coordination, and follow-up scheduling.
- Help develop and maintain standard operating procedures, workflows, educational materials, and quality-improvement processes.
- Monitor program data and prepare reports related to enrollment, appointment adherence, medication access, patient retention, and clinical outcomes.
- Coordinate intake, scheduling, and follow-up for patients receiving long-acting injectable services.
- Assist in finalizing LAI workflow and SOP with supervisor/PrEP Retention Manager.
- Help train and support new and existing PrEP retention team on LAI procedures and workflows or other updates needed.
- Educate patients about treatment and prevention options, appointment expectations, medication schedules, laboratory requirements, and follow-up care.
- Support shared decision-making and provide nonjudgmental, trauma-informed, sex-positive, and LGBTQIA+ affirming service.
- Coordinate medication benefits investigations, prior authorizations, patient assistance programs, pharmacy arrangements, and medication delivery or pickup.
- Track injection schedules, laboratory results, missed appointments, medication status, and follow-up needs.
- Maintain accurate and timely documentation in the electronic health record and other required tracking systems.
- Communicate with clinicians, nurses, pharmacists, laboratories, pharmacies, insurers, community partners, and patients to support continuity of care.
- Conduct appointment reminders and outreach to patients who miss appointments or are at risk of being overdue for medication.
- Assist with resolving barriers related to transportation, insurance, cost, language access and stigma
- Protect patient privacy and comply with HIPAA, organizational policies, and applicable federal, state, and local regulations.
- Perform other duties as assigned.
Key Performance Indicators
- Initial KPIs: Finalize LAI workflow, SOPs, and successfully train new & existing PrEP Retention staff on workflow and SOPs.
- Future KPIs: Will be provided once a baseline of productivity has been established.
Based on the position’s duties and requirements, this role has a base hourly rate of $27.82 The initial salary will be determined by the selected candidate’s qualifications and relevant experience.
Requirements
Required
- High School Diploma, Associate degree preferred or equivalent combination of education and relevant experience.
- At least two years of work experience in healthcare coordination, case management, patient navigation, public health, sexual health, HIV services, pharmacy services, or a related field.
- Six (6) months of work or volunteer experience with marginalized communities, such as LGBTQIA+ and communities of color.
Preferred
- Experience coordinating injectable medications, HIV PrEP or treatment, STI services, gender-affirming care, hepatitis services, or other specialty medications.
- Experience with insurance authorization processes, patient assistance programs, specialty pharmacies, and medication access resources.
- Experience with patient retention and medication management/adherence.
- Bilingual fluency in both English and Spanish, including knowledge of medical vocabulary.
- Experience as a Medical Assistant, Community Health Worker, Patient Navigator, Case Manager, or similar credentials.
KSAs
- Strong organizational, communication, documentation, and follow-through skills.
- Knowledge of insurance authorization processes, patient assistance programs, specialty pharmacies, and medication access resources.
