Job Summary:

The Hospice Clinical Liaison plays a dual role that combines clinical nursing expertise with strategic relationship-building. Serving as both an admissions nurse and a community liaison, this role is responsible for assessing patients for hospice eligibility, facilitating timely and compassionate admissions, and developing strong partnerships with referral sources to grow awareness and trust in our hospice services. 

 

Key Responsibilities: 

  • Conduct timely and thorough clinical assessments to determine hospice eligibility based on established criteria.
  • Collaborate with the interdisciplinary team to develop patient-centered plans of care.
  • Educate patients and families on the hospice philosophy, services, and what to expect throughout their care journey.
  • Ensure all admissions documentation is completed accurately, timely, and in compliance with federal, state, and agency standards.
  • Maintain up-to-date knowledge of hospice guidelines, insurance policies, and community resources.
  • Provide emotional support and guidance to patients and families during the transition to hospice care.
  • Build and maintain strong relationships with hospitals, skilled nursing facilities, physicians, and community healthcare partners.
